4k. Informacja, o której mowa w ust. 4j pkt 2 lub 3, zawiera oświadczenie podmiotu, czy:

1) urządzenia lub systemy teleinformatyczne, o których mowa w ust. 4j pkt 2, umożliwiają identyfikację osoby uzyskującej informacje z Rejestru, zakresu informacji oraz daty ich uzyskania;

2) wdrożone zabezpieczenia techniczne i organizacyjne, o których mowa w ust. 4j pkt 3, zapewniają poufność, integralność, dostępność i autentyczność przetwarzanych danych.
