4. W przypadku, o którym mowa w ust. 3 pkt 2 i 3, do wniosku dołącza się kopię:
1) dokumentu potwierdzającego reprezentację podmiotu niepublicznego,
2) pełnomocnictwa do złożenia wniosku w imieniu podmiotu niepublicznego, jeżeli pełnomocnik został ustanowiony
– potwierdzone za zgodność z oryginałem przez wnioskodawcę albo przez notariusza, albo przez pełnomocnika wnioskodawcy będącego adwokatem, radcą prawnym, rzecznikiem patentowym lub doradcą podatkowym, jeżeli wniosek nie został opatrzony kwalifikowaną pieczęcią elektroniczną.
