9. Wniosek o przeprowadzenie ponownego egzaminu składa się niepóźniej niż przed upływem roku od dnia egzaminu, z którego kandydat uzyskał wynik negatywny. Wniosek ten zawiera:

1) imię i nazwisko kandydata;

2) adres zamieszkania kandydata;

3) oznaczenie postępowania w sprawie stwierdzenia kwalifikacji, w którym kandydat został dopuszczony do egzaminu.
