2. Wniosek o zmianę koncesji, poza wymaganiami przewidzianymi przepisami z zakresu ochrony środowiska i działalności gospodarczej, zawiera szczegółowe uzasadnienie proponowanych zmian. Do wniosku dołącza się dowody istnienia określonych w nim okoliczności, a w przypadku gdy zmiana koncesji wpływa na warunki umowy o współpracy, dołącza się także zmianę tej umowy.
