3. Do zgłoszenia, o którym mowa w ust. 1, dołącza się:

1) dowody na podjęcie działań zmierzających do realizacji zamiaru, o którym mowa w ust. 1, w szczególności umowy lub porozumienia odnoszące się do tego zamiaru;

2) dane identyfikujące podmiot przejmujący:

a) firmę przedsiębiorcy, oznaczenie jego siedziby i adresu albo miejsca zamieszkania i adresu oraz adresu głównego miejsca wykonywania działalności gospodarczej,

b) numer w rejestrze przedsiębiorców w Krajowym Rejestrze Sądowym, o ile przedsiębiorca taki numer posiada, oraz numer identyfikacji podatkowej (NIP);

3) wykaz koncesji, które mają przejść na podmiot przejmujący w wyniku dokonania czynności, o których mowa w ust. 1;

4) dowody, że podmiot przejmujący spełnia warunki, o których mowa w art. 36 ust. 1 i 6, a w przypadku koncesji na podziemne składowanie dwutlenku węgla – również warunki, o których mowa w art. 36 ust. 3a.
