6. Wniosek o wydanie dopuszczenia zawiera:
1) określenie wyrobu;
2) oznaczenie podmiotu ubiegającego się o wydanie dopuszczenia, przez wskazanie jego formy prawnej oraz dowodu jego istnienia, w szczególności wypisu ze stosownego rejestru, oraz jego siedziby, a także osób upoważnionych do działania w jego imieniu, przez podanie ich imienia i nazwiska oraz stanowiska służbowego;
3) określenie producenta wyrobu, jego siedziby i miejsca produkowania wyrobu.
