Art. 584⁵. Do przekształcenia przedsiębiorcy wymaga się:

1) sporządzenia planu przekształcenia przedsiębiorcy wraz z załącznikami oraz opinią biegłego rewidenta;

2) złożenia oświadczenia o przekształceniu przedsiębiorcy;

3) powołania członków organów spółki przekształconej;

4) zawarcia umowy spółki albo podpisania statutu spółki przekształconej;

5) dokonania w rejestrze wpisu spółki przekształconej i wykreślenia przedsiębiorcy przekształcanego z Centralnej Ewidencji i Informacji o Działalności Gospodarczej.
