§ 2. Urlop ojcowski jest udzielany na wniosek w postaci papierowej lub elektronicznej składany przez pracownika – ojca w terminie niekrótszym niż 7 dni przed rozpoczęciem korzystania z urlopu. Do wniosku dołącza się dokumenty określone w przepisach wydanych na podstawie art. 1868a. Pracodawca jest obowiązany uwzględnić wniosek pracownika.
