Art. 529.

§ 1. Wniosek o uznanie za zmarłego, oprócz danych koniecznych dla wniosku o wszczęcie postępowania, powinien zawierać:

1) imię, nazwisko i wiek zaginionego, imiona jego rodziców oraz nazwisko rodowe matki;

2) ostatnie znane miejsce zamieszkania i pobytu zaginionego.

§ 2. Okoliczności uzasadniające wniosek należy uprawdopodobnić.
