Art. 312. Wniosek powinien zawierać:

1) oznaczenie wnioskodawcy i przeciwnika oraz innych osób zainteresowanych, jeżeli są znane;

2) wskazanie faktów oraz dowodów;

3) przyczyny uzasadniające potrzebę zabezpieczenia dowodu.
